This authorizes the Cottonwood Canyons Foundation and its payroll processor, GUSTO, (together referred to as the “Company”) to send credit entries (and appropriate debit and adjustment entries), electronically or by any other commercially accepted method, to my (our) account(s) indicated below and to other accounts I (we) identify in the future (the “Account”). This authorizes the financial institution holding the Account to post all such entries. I agree that the ACH transactions authorized herein shall comply with all applicable U.S. Law. This authorization will be in effect until the Company receives a written termination notice from myself and has a reasonable opportunity to act on it.

Cottonwood Canyons Foundation Vehicle Use Policy
For Use of Organization Vehicles

Drivers must have a valid driver's license of the State they reside in.
Drivers should have at least two years driving experience and be at least 18 years old.
Drivers must present a Motor Vehicle Record for all states in which the Driver has lived in the previous four (4) years. MVRs for the State of Utah can be ordered here https://dld.utah.gov/mvr-online
In the past three (3) years, drivers should have no more than:
1. Two (2) At-Fault Accidents, (Proof of not-at-fault status for accidents must be received to rescind this. Your personal auto insurer or DMV can assist with this information.)
2. Three (3) Minor Moving Violations, (personal and/or business)
3. Four (4) of the following Non-Moving Violations such as:
  - Failure to Appear (FTA),
  - License not in possession, or
  - No Proof of Insurance/Registration
4. Four (4) or more in combination of the above a, b, and c.
In the past four (4) years, drivers should have zero (0) Major Moving Violations including, but not limited to:
- Driving while Suspended/Revoked and/or Invalid License
- Exhibition of Speed - Speed Contest
- Reckless Driving
- Driving Under the Influence (DUI)
- Vehicular Manslaughter
- Leaving the Scene of an Accident, (Hit and Run)
- Speeding in excess of 100 mph, etc.

Dear Cottonwood Canyons Foundation directors:

By signing this letter, I agree that I have read the Cottonwood Canyons Foundation Vehicle Use Policy and am currently in full compliance. I am providing CCF with a copy of my applicable Motor Vehicle Records in accordance with the policy and do so voluntarily. If, at any time from today's time onward, I become aware that I am no longer in compliance with the Vehicle Use Policy, I will immediately inform my supervisor and will cease driving any Organization Vehicle.
