Please fill out this form to reserve a day pass(es) for pickup. Requested passes are stocked for pickup each week on Wednesday afternoon. Your pass(es) will be stocked for pickup the Wednesday after you have submitted a reqeust.

If you are reserving day passes for more than one resort for pickup, you will have to submit this form once for each resort you intend to reserve day passes for.

Reserved day passes will be available for pickup from CCF's office lockboxes the Thursday after reservations have been submitted.

Lockbox codes will be provided for day pass pickup.

Passes will be located at our CCF office in an outdoor lockbox. The CCF office is located at 2091 E Murray Holiday Road, Holladay. Use the lockbox code(s) provided to access your passes. Passes for Big Cottonwood Canyon will be placed in the lockbox labeled BCC, while passes for Little Cottonwood Canyon will be placed in the lockbox labeled LCC.

You may only take the pass(es) specifically marked out for you.

To use the day passes provided to us by Alta, Snowbird, Brighton, and Solitude, you must read and sign this form. By doing so, you agree to the terms described below. Violation of these terms will result in the forfeiture of your ski pass benefits and possible expulsion from CCF's volunteer programs.

• - You must sign this form before you can redeem day passes.
• - You may not request more day passes than unspent volunteer days you, or the entitled volunteer has accrued.
• - Day passes provided by CCF may only be used by CCF volunteers and their accompanying guests. You may not sell or attempt to sell your benefit day passes under any circumstance. These passes are coded specifically for SWR/SWN and must be used within the context of this program.
• - Requested benefit day passes will be stocked in the drop boxes on Wednesday afternoon. Consequently, day pass requests made before Wednesday afternoon can be picked up the next day (Thursday), but day pass requests made after Wednesday afternoon (including Wednesday evening) will not be available until the following Thursday.
• - Day passes may only be picked up by the individual who has reserved them. You may not pick up any day passes that are not specifically reserved for you.
• - If you will not be using a pass that you have reserved, please contact Liz ASAP so that no passes are wasted.
