IN CONSIDERATION OF BEING ALLOWED TO PARTICIPATE IN PROGRAM RELATED EVENTS AND ACTIVITIES, I THE UNDERSIGNED, ACKNOWLEDGE, APPRECIATE AND AGREE THAT: Risk of injury from activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERS, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation. If I observe any unusual significant hazard during my participation, I will remove myself from the hazard and bring the hazard to the attention of the nearest CCF official immediately and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE COTTONWOOD CANYONS FOUNDATION, their officials, board members, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (RELEASEES), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss of damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

Photo Release: I authorize CCF to: take photographs and/or video clips for the purpose of production, whether by film, videotape, magnetic tape, digitally or otherwise; make copies of the photographs and recordings so made; use my likeness for the purposes of education, promotion or advertising of the programs and services of CCF. I extend this permission to legitimate partners of CCF. There will be no restrictions on the number of times my likeness may be used. Also, unless otherwise stated, there will be no restrictions to the geographical distribution of images. I acknowledge that I am not to be compensated for the use of my image. I understand the terms described in this waiver. I am over 18 years of age and have the authority to sign this contract and grant the rights given under this waiver or if I am a minor my parent/guardian has signed below.
